SPH 9: Notification that Information Will Not be Corrected
SPH 9: Notification that Information Will Not be Corrected is used by a previous employer to notify a DOT-regulated driver that his or her safety performance history information will not be corrected. When a driver believes that the safety performance history information provided by a previous employer contains errors, he or she may request a correction in writing. A prior employer uses SPH 9 to notify the driver that his or her request has been denied.
