SPH 2: Driver’s Written Authorization to Release Information

SPH 2: Driver’s Written Authorization to Release Information is used by prospective employers to demonstrate that a FMCSA-regulated driver-applicant authorizes the release of previous employment and safety performance history as required by 49 CFR Part 391.23. The driver-applicant must complete separate release forms for each previous employer who will be contacted as part of the mandatory safety performance history background investigation.
